Working within the area of Special Works, the Family Services Case Manger will support people who are homeless/at risk of homelessness or experiencing social disadvantage in the ACT. Working as the Case Manager, you will play a key role in the delivery of case management to families and engage, assess and support people via holistic, sustainable case management plans, meeting the outcomes required of the program and the person. You will assist in building individual capacity to enable people to live a fulfilling life, helping them to make optimum choices, enhancing their living skills and confidence, and gaining access to mainstream services.
To be successful in this role, you must view it as more than just a job, demonstrating a proven ability to navigate diverse programs and client demographics. An in-depth understanding of the challenges faced by individuals experiencing homelessness and disadvantage is imperative. You should possess the capability to effectively prioritize a complex and varied caseload to achieve desired outcomes, while working with minimal supervision.
In addition, strong communication skills are essential, coupled with a proactive, collaborative mindset. Demonstrating resilience and the ability to adapt to change, even in challenging environments, is crucial. A bachelor's degree in human services or a related discipline, such as psychology or social work, is required, along with a minimum of two years' experience supporting vulnerable populations, particularly youth and families, through community-based case management.
You will be required to undergo and maintain a clear police record check and successfully hold or obtain a Working with Vulnerable People Card (WWVP). Current drivers’ license and current first aid certificate (or the willingness to obtain within three months of employment) is also mandatory.
