About the Role:

As the Access and Support Coordinator, you will play an essential role in VincentCare's vital Access and Support program, which is dedicated to connecting people under the age of 65 with health, mental health, disability and other community-based support services. Utilising case management frameworks and client-based approaches, you will undertake direct support and care coordination to assist clients clients in accessing mainstream and specialist services.

The role will also work closely with other teams in VincentCare's Northern Community Hub services in assisting people who are homeless, at risk of homelessness and in low cost accommodation, or those who are financially disadvantaged.

About you:

You are a qualified professional with experience in providing comprehensive case management, planning and support to assist clients within a homelessness setting. You have experience in maintaining constructive, empathetic, supportive and respectful working relationships particularly for an integrated service response.

Key Selection Criteria:

  • Minimum of Diploma level, Tertiary Qualifications in Community Services or related field;
  • Experience in supporting people with planning, linkages and support in an outreach model;
  • Demonstrated understanding of and demonstrated ability to support the range of needs and issues affecting people who are homeless or at risk of homelessness. Including mental health, AOD, disability, justice etc;
  • Ability to work collaboratively within a team and contribute to consistent, yet innovative work practises;
  • Demonstrated ability to effectively build, engage and maintain professional working relationships with internal and external stakeholders;
  • Evidenced administrative accuracy and detail, including timely data entry.
