About the Role

In this position you will:

• Increase the uptake of healthcare by Elisabeth Street HomeGround (ESCG) residents
• Breakdown barriers to access to healthcare for ESCG residents
• Empower residents to manage their own health
• Assist residents to live safer and more sustainable lives in their home and community.
• Demonstrate that an expanded primary health care and care coordination intervention in homelessness services can improve health outcomes for people experiencing / at risk of homelessness.

Your duties:

• Work collaboratively with the Launch Housing team to foster a joint understanding of the primary health service at the site
• Provide community based clinical nursing assessment of acute and chronic health needs of residents and support residents through a care coordination approach to develop goals and achieve improved health outcomes.

Classification for this position is YX11 ($113,646 per annum, pro rata for part time).

Your Contribution

• You are an AHPRA Registered Nurse, with 7 years’ experience in acute/community setting.
• Advanced clinical skills and experience in an area of mental health, alcohol and other drug use, working with residents with complex psychosocial needs.
• Advanced skills in working with and identifying clinical risk, and escalating to appropriate level of clinical care and internally via SVHM Team Leader/Manager.
• Experience with prioritising residents support needs
• Experience operating in an outreach capacity with residents with complex needs.
• Experience in operating within a multidisciplinary team and or with staff from non-health backgrounds.
• National Police Check
• Current Victorian Driver’s Licence

About Healthcare for the Homeless

This position sits within the SVHM Healthcare for the Homeless program and will work in partnership with Launch Housing. Whilst the role will primarily be embedded within ESCG, it will also maintain close professional links with SVHM Healthcare for the Homeless department and related services..
