About the Role

This particular position has two components:

As a Care Coordinator you will assess eligible, high-risk clients living with complex healthcare needs. The primary aim of this component of the position is to provide generic care coordination that facilitates the client’s self-management strategies in order to improve health outcomes and prevent avoidable hospital presentations. Intervention is provided through a collaborative care coordination approach and will integrate traditional hospital based health care services and primary health care. This role includes screening referrals, recruiting clients onto the program, conducting comprehensive assessments, coordinating all aspects of health and social care needs, monitoring achievement of client goals, facilitating intervention from other health care professionals and ensuring appropriate linkages with hospital and community services.

As a Dietitian, you will provide targeted dietary assessment, intervention, lifestyle coaching and clinical management of dietary related conditions and health behaviours. Dietetic intervention will support the health, wellbeing and self-management of any client accessing HIP services. The role may also include the prescription of nutritional supplements, complementary therapies, provision of material aid to improve access to food and working alongside clients to increase food security. You will be working with clients that have complex comorbidities, psychosocial issues and financial difficulties that all impact on their nutritional status.

This position will receive clinical supervision directly from the Grade 3 Complex care Dietitian and strong links with the Dietetics department.

Classifications for this position will be Dietitian grade 2 Year 1 - 4 AJ1 - AJ4 ($90,812.80 - $102,757.20 per annum, pro rata for part time) depending on experience.

Your Contribution

  • Bachelor of Nutrition Science
  • Minimum 5 years experience in relevant acute/community setting
  • Eligible for registration under the Accredited Practicing Dietitians.
  • Experience with prioritising patients and hospital demands
  • Demonstrated sound knowledge of the application of care coordination practice frameworks and principles, in the context of the current health care environment.

Health Care for homeless Program

This position is part of the Health Care for homeless Program (HfH). The program has a base of services within St Vincent’s that currently support clients that are experiencing or at risk of homelessness. This service aims to build on integrating and better coordinating existing services within St Vincent’s hospital and the development of new services to meet the need of the target client group.

Operationally the role has strong links with the Health Independence Program (HIP). HIP offers a comprehensive range of specialist clinics, services and rehabilitation programs, providing rapid response health management, short-medium term care coordination and health education. HIP enable clients to be the decision makers of their care, and support clients through self –management to achieve their healthcare goals.

ALERT key target groups include patients with complex needs who fall under one or more of the key risk factors; substance use, mental health issues, homelessness, disability and complex aged care issues, family violence.

This position is based at St Vincent’s Hospital Melbourne within the Emergency Department and also providing support and interventions in the community as part of an interdisciplinary team.

Hours of Duty:

  • 08:30 – 17:00 as per 7 day week roster
  • Weekends and public holidays as rostered
  • Other extended hours may be required as per roster requirements
