St David's Neighbourhood Centre is a not for profit organisation and is situated at Coopers Plains.

St David’s Neighbourhood Centre deliver programs and services that support people in our community. These include Stay nPlay (Playgroup), Social & Educational programs (funded by Dept. Communities, Child Safety & Disability Services), Thrift Shop and Harmony Club – programs for frail & older clients (Department of Health) & Adults with a disability (NDIS).

Primary Role:

Due to growth within our program, we are seeking a compassionate and enthusiastic person to join our team of support workers to work in our Harmony Club program. The position reports directly to the Harmony Club Coordinator and works with others in the provision of a broad range of activities for older adults and those with a disability experiencing social isolation and diminishing physical, intellectual and social capabilities. A person-centred approach is taken to the planning of services and programs that are provided through our day respite centre and social support activities.

Support workers assist clients to access their community through social support activities such as social outings, community events such as concerts, shopping, and socialising with others. Support workers also facilitate clients to participate in the day respite program at the centre and the activities on offer. The position assists with leave coverage for other support works and the Food Services Supervisor which includes preparing and cooking meals for up to 20 clients per day.

Responsibilities:

Harmony Club Support Workers are responsible for:

  • Transport of clients to and from the centre and in the community
  • Planning and implementation of social support and day respite activities in conjunction with the Harmony Club Coordinator
  • Encourage and assist clients to participate in activities both within the centre and community
  • Assist clients with their personal care needs and to purchase meals and refreshments when in the community.
  • Completion of client documentation including progress notes, incident forms etc.
  • Provide supervision and direction to volunteers they may be working with in the completion of daily activities

The right person will have or be:

  • Experience in working with frail aged and those with a disability in a community setting
  • Experience in the planning, implementation and facilitation of group and social support activities
  • Qualification in Aged Care/Community/Disability
  • Current open QLD Drivers Licence and the ability and confidence to drive an 11 seater bus
  • A current and valid National Criminal History Check Certificate and positive notice Blue Card & Yellow Card (or be willing to apply for these)
  • Current First Aid and CPR certificate

  • Willingness to listen to others
  • Able to work with limited supervision and as part of a small team
  • Effective written and oral communication skills
  • Willingness to work within the ethos of the Uniting Church

Remuneration is SCHADS Award level 2.
