Join Soulmate Care

Soulmate Care Services is seeking an experienced Community Worker to provide structured support to people with disability, older Australians and individuals facing social, emotional or functional barriers.

This is a permanent full time role based in Brisbane.

The role focuses on helping clients live more independently, stay connected with their community and access the right services at the right time. You will assess client needs, prepare practical support strategies, coordinate services, monitor progress and work closely with families, carers and community organisations.

Key responsibilities include:

  1. Assessing client needs, strengths, circumstances and support requirements.
  2. Developing and implementing individual support plans that promote wellbeing, independence and inclusion.
  3. Helping clients access health, housing, education, training, employment and community services.
  4. Supporting clients to participate in community, social, recreational and daily living activities.
  5. Liaising with families, carers, service providers and community organisations.
  6. Providing guidance, advocacy and referrals to appropriate welfare and community services.
  7. Monitoring client progress and identifying changes in support needs.
  8. Maintaining accurate case notes, progress records and service reports.
  9. Contributing to service planning, outcome reporting and continuous improvement.

Ideal Candidate:

  1. Holds an NDIS Worker Screening and Blue Card.
  2. Has a current First Aid and CPR certificate.
  3. Possesses a valid driver’s licence and access to a reliable vehicle.
  4. Can provide a National Police Check.
  5. Holds a Working with Children Check (WWCC).
  6. Diploma in Community Services or a related field.
  7. Familiar with the NDIS system and processes.
  8. Strong interpersonal skills for engaging with clients, families, and stakeholders.
  9. Ability to maintain detailed case notes and prepare service reports.
  10. Experience collaborating with allied health professionals, NDIS providers, and agencies like Centrelink.
