This role is within our care finder services located across selected Western LGAs on behalf of Western PHN and the Commonwealth Department of Health and Aged Care.
Joining us, you’ll guide a small team to provide service delivery of Social Futures care finder services across Dubbo, Narromine, Gilgandra, Warrumbungle and Mid-Western regions. This program is designed to provide specialist and intensive assistance to help people to understand and access aged care and connect with other relevant supports in the community.
