We currently have an exciting opportunity for a Pharmacist to join our team as a Medication Safety Pharmacist Research Assistant within the ASPIRE Project and be a part of a dynamic team working with Sydney North Health Network (Northern Sydney Primary Health Network), Royal North Shore Hospital (Northern Sydney Local Heath District) and the University of Sydney.

ASPIRE (A Systems aPproach to enhancing community-based medIcation REview) is a multi-centre transition of care study to investigate whether a model for improving access to post-discharge medication reviews, namely Home Medicines Review (HMR) or Residential Medication Management Review (RMMR), reduces readmissions. It is being led by Professor Timothy Chen, School of Pharmacy, the University of Sydney, and a team of investigators affiliated with the University of Sydney, University of South Australia, University of Newcastle, Royal Brisbane and Women’s Hospital and Curtin University. It is funded under a Medical Research Future Fund grant (MRFQI000043).

The study is being conducted at four hospitals and corresponding Primary Healthcare Networks (PHNs) in New South Wales and Queensland:

  • Royal North Shore Hospital with Northern Sydney PHN
  • John Hunter Hospital with Hunter New England and Central Coast PHN
  • Broken Hill Hospital with Western NSW PHN
  • Royal Brisbane and Women’s Hospital with Brisbane North PHN

The Medication Safety Pharmacist Research Assistant will carry out clinical trial research activities for a multi-centre transition of care study that is investigating whether a model to improve access to timely post-discharge medication reviews reduces readmissions.

The role will contribute to practice and services development for improving patient outcomes during the transition of care.

In addition, Medication Safety Pharmacist Research Assistant will:

  • have a key role in the implementation model for the trial, which is to establish a Medication Safety Hub in the PHN.
  • be responsible for implementation of the intervention (model) and for data collection follow-up of patient participants in the trial.
  • require a sound knowledge of medication review services (MMR accreditation is desired), an ability to establish and foster productive relationships with a range of healthcare stakeholders, expertise in communicating effectively with general practitioners and pharmacists, and experience in managing clinical data.

Location

The position will be based at the Sydney North Health Network office in Chatswood with flexible, hybrid working arrangements available, in agreement with line manager.
