There is an exciting opportunity to join the Chronic Disease Team working with vulnerable clients in the community. The position is shared between the Primary Care at Home team and the Nurse-led St Patricks Health Clinic in Fremantle. You will be supported by our team of Co-ordinators, GP's and Program Manager.
Primary Care at Home is a community outreach program which seeks to engage and offer holistic primary health care to vulnerable and disadvantaged adults across Metropolitan Perth. This role involves working alongside clients with chronic disease/s using the Flinders Chronic Disease Self-Management Program. You will also provide health literacy, advocation and linkage to community, generalist, and specialist services in the South region.
St Patrick’s Community Support Centre, a not-for-profit specialist service in Fremantle supporting vulnerable people in the community who are homeless, or at risk of homelessness. The role involves working 2 days a week in the nursing clinic in Fremantle providing clinical assessment, chronic disease management, wound care, diabetic care, health education, promotion, navigation, and referral to other services and 1 day working in the primary care at home south Perth Metro.
You will be an experienced, multi-skilled Nurse passionate about reducing barriers to primary health for vulnerable populations. We are looking for skilled individuals who bring warmth, empathy, and inclusivity in their role to reconnect people back to health and community, Using your chronic disease knowledge, you will support the person to better manage their conditions by providing health literacy, case management, advocacy, and health service navigation.
