The team operate locally as self-organised teams - called Recovery 2gether teams. Team members are collectively responsible and accountable for:
Team members work collaboratively with other team members to facilitate the effective operation of the team supported by an Operations Lead/Recovery 2gether Coach.
The Bilingual Hospital to Home (H2H) supports people living with mental illness, and people who have self-harmed or attempted suicide; for up to 12 weeks following discharge from hospital. We help people to connect, to live well in their community.
The Bilingual Mental Health Worker is responsible for building relationships and capacity within the Western Sydney Community to strengthen co-ordination, referral pathways and connections between GPs, Allied Health Providers, Government Agencies, Health Services and Community Services to improve support for individuals from culturally and linguistically diverse populations who are impacted by severe and persistent mental health conditions.
