Sunbury and Cobaw Community Health is a not-for-profit community health organisation providing a comprehensive range of free and low-cost health, wellbeing and community services in early childhood, youth, families, adults, aged and disability.

About the role:

Our Primary Health Unit is seeking an experienced Physiotherapist to join our multidisciplinary team to work with individuals and groups aged 18 years and over based in Kyneton.

The position is responsible for providing quality community-based physiotherapy services, encompassing a diverse range of health conditions including musculoskeletal and chronic disease management.

About you:

You will have demonstrated experience in applying the principles of chronic disease management, client directed care planning, client goal setting and self-management. You will also have a clear understanding and commitment to the Social Model of health and Person-Centred Care

The successful candidate will be required to undertake and maintain a National Criminal History Check (NCHC), an NDIS Safety Screening check and hold a current valid Working With Children Check. As a healthcare provider, all our staff will need to be vaccinated for COVID-19 or have a medical exemption by an authorised medical practitioner.
