Sunbury and Cobaw Community Health is a not-for-profit community health organisation providing a comprehensive range of free and low-cost health, wellbeing and community services in early childhood, youth, families, adults, aged and disability.

Covering an area of over 1700 km2 from Sunbury to beyond the borders of Macedon Ranges Shire our 280 staff and 100 volunteers are based across four sites at Kyneton, Romsey, Sunbury and Woodend.

The organisation has a proud history of delivering community health services to the region for over 40 years.

About the role:

Our Community Connections Unit is seeking a Support Coordinator for our NDIS Support Coordination program. The preferred applicant will have experience working in the NDIS & Disability sector using the principles of consumer directed care.

The primary function of this role is to assist NDIS participants to activate their NDIS plans. This includes supporting participants to understand their plan, understand NDIS processes, identify service options, and manage their allocated individual budget.

You will be an experienced and passionate Support Coordinator committed to promoting consumer directed care.

Applicants will have experience working with clients who present with diverse needs. Excellent communication, planning and problem solving skills are essential. The position may include travel between sites in Sunbury and the Macedon Ranges.

You are an experienced and hardworking NDIS professional committed to promoting consumer directed care and supporting NDIS Participants achieve their goals and want to be part of our NDIS Support Coordination Program.

You Demonstrate:

  • Empathy and a genuine interest in the outcomes for people living with a disability and the capacity to be creative in responding to assist participants to achieve their goals.
  • Relevant knowledge and experience in case management within the health, disability, or community sectors.
  • Knowledge of the NDIS or demonstrated ability to develop this knowledge.
  • Ability to develop strong working relationships with service providers.
  • A commitment to working from a strengths-based approach with participants.
  • Enjoy being part of our NDIS Support Coordination Program.
