About the role

In this exciting new and growing role, you'll be reporting to our Home Care Manager.

You will manage a case load of Home Care Package Residents living within our Villages and ensure the smooth running of end-to-end customer experience, with a focus on ensuring the wellbeing of older people who are in receipt of a Home Care Package.

You will, action new leads & enquires, assess resident's care needs, develop care plans, monitor home care budgets, and remain within allocated funding, while working collaboratively with care staff and monitor care provided with a strong focus on customer satisfaction and maximising outcomes for residents with a Home Care package.

This role will be a mobile based role, with extensive time spent supporting our residents within our villages around Metropolitan Melbourne (South East Suburbs).

About you

You will be passionate about making a difference to our residents lives and provide outstanding customer service, and;

  • Case Management experience within aged care, or an understanding of Consumer Directed Care within Home Care Packages, as well as an understanding of HCP guidelines and standards
  • Tertiary qualification in Health, community services, aged care, or disability
  • Registered Nurse highly desirable
  • Have a warm and friendly approach to build lasting relationships with our residents and their families
  • Exceptional organisation, problem solving, computing, client database management and time management skills
  • Excellent written, verbal, and numerical communication skills
  • Current driver's licence
