This role requires collaboration with customers, coordinators, brokered partners, and colleagues to ensure a comprehensive approach to care planning and scheduling. Positioned under the guidance of the Service Manager, the role is integral to the care delivery framework, and champions the 'Connect' model of care principles through diligent customer-focused scheduling and engaged participation within the care team, thereby facilitating a care experience that is both personalised and impactful.
