Peninsula Home Hospice is a not for profit organisation providing specialist palliative care in the home. Care is provided from South Mordialloc through to the Mornington Peninsula. We are seeking a compassionate, enthusiastic Clinical Leader with Division 1 nursing and post graduate qualifications in palliative care.

This key role works closely with the Clinical Services Manager and clinical nurse consultant in providing leadership to the nursing team to ensure effective specialist palliative care nursing systems and processes.

Working to achieve service delivery goals through professional leadership as a member of an interdisciplinary team, and to provide direct client care through assessment, ongoing care planning and evaluation. The role requires a commitment to positively influence service delivery outcomes for clients and their caregivers, along the palliative care trajectory.
