What You Will Be Doing

The Transition Care Program (TCP) is seeking an experienced, innovative and flexible individual to work as a member of the TCP Physiotherapy team.

The program is a jointly funded program (DHS/Commonwealth) and it aims to assist frail aged patients reach their maximum functioning capacity and to help determine their long term care arrangements.

TCP operates in both the Community as well as two Residential Aged Care environments, offering scope for clinicians to enhance their experience along the care continuum.

Please note the following:

  • Flexible working environment with options to do some work from home
  • Great varied caseload across inpatient residential and home-based settings – no getting stuck in an clinic room or on a ward
  • Opportunity to develop higher level prioritisation, delegation and caseload management skills
  • Great multidisciplinary close knit team

The successful applicant will need to be flexible in terms of working across both residential and community areas. The Physiotherapist will need to demonstrate sound clinical judgement, assessment and practice skills, and have an understanding of the issues affecting patients with complex functional and cognitive needs and an interest in aged care issues.

The successful applicant will need to be highly motivated, and enthusiastic and to be able to work both independently and as part of a team.

What You Need

We anticipate you will have:

  • Eligibility/Membership of relevant professional body
  • Experience in a sub-acute and/or community setting
  • Capacity to supervise student
  • Knowledge of / experience in Aged/Residential Care settings.
  • Current Driver’s Licence
  • Demonstrated participation in Professional Development.
  • Interest and/or experience in Quality improvement and research activities
  • Computer literate
