About the Role
We’re seeking an experienced Outreach Worker to engage with young people who may be disengaged, isolated, or at risk. This role combines outreach and NDIS-funded support—meeting participants where they are, building trust, and helping them achieve their personal goals.
You’ll work across home, school, and community settings to provide mentoring, coaching, and capacity-building supports that align with participants’ NDIS plans.
Key Responsibilities
- Build safe, trusting relationships using trauma-informed practice
- Deliver person-centred NDIS supports that promote independence and skill development
- Support participants to access education, employment, health, and housing services
- Provide mentoring, coaching, and community engagement activities
- Identify and respond to risks or wellbeing concerns
- Maintain accurate case notes and communicate effectively with families and providers
About You
- Qualifications in Youth Work, Social Work, or Community Services, plus current First Aid, WWCC, and NDIS worker screening check.
- Experience working with vulnerable, disengaged, or NDIS-participating young people (minimum 6 months)
- Strong communication, empathy, and relationship-building skills
- Understanding of NDIS principles, trauma-informed and strengths-based practice
- Ability to work independently in community settings
- Commitment to inclusion, safety, and the rights of young people
