Job Description

Generally, a Service Coordinator's job description may include:

Intake Interviews: The service coordinator will conduct intake interviews of individuals with disabilities to identify their needs, strengths, and preferences.

Risk Management: The service coordinator will work with individuals with disabilities to identify, classify, manage, control and monitor those risks that threaten the Services provided and manage all associated records so there is up to date and accurate documentation of the risk assessment at all times. It is an ongoing process that requires the development of cost effective control measures to reduce risks and maximise opportunities.

Developing Individualised Support Plans: The service coordinator will work with individuals with disabilities to assess their needs and develop individualised service plans that outline the specific services, interventions, and supports required to help them achieve their goals.

Coordinating Services: The service coordinator will work with service providers, healthcare professionals, and other stakeholders to ensure that people with disability have access to the resources and support services they need.

Rostering and Shift Notes Review: The service coordinator will allocate work shifts and duties to support workers based on their availability, skills, and clients' requirements. In addition, the coordinator will monitor staff attendance, performances, and take appropriate action to address any issues that may arise. The coordinator will also review the progress notes to check critical details about client's services.

Advocating for Individuals: The service coordinator will advocate for people with disability to ensure that their rights are protected and that they receive the services and support they need to live independently.

Monitoring Progress: The service coordinator will regularly monitor and evaluate service delivery to ensure that people with disability are receiving high-quality services that meet their needs and expectations. ​

Maintaining Records: The service coordinator will maintain accurate and up-to-date records of all service delivery activities, including assessments, support plans, progress reports, and other documentation.

Providing Guidance and Support: The service coordinator may also provide guidance and support to family members, caregivers, and other individuals involved in the individual's care.

Identifying Resources: The service coordinator is responsible for identifying resources within the community that can support the individual's needs and goals. ​​

Incident Management: The service coordinator will swiftly and efficiently respond to, manage, and escalate an issue, bringing it under control and minimising the impact on the participant and PandL.

Ensuring Compliance: The service coordinator ensures that all services provided are compliant with federal, state, and local regulations. The service coordinator may participate in review of the organisation's policies and processes ensuring alignment with the organisation's standards. Likewise, may contribute to continuous improvement culture that reviews indicators of client satisfaction and implement action to achieve results.​

Training and Development: The service coordinator may provide ongoing support and coaching to people with disability to help them achieve their goals and improve their overall quality of life. Similarly, service coordinator may provide training and development opportunities to disability support workers (DSW) to ensure that they have the necessary skills and knowledge to support people with disability effectively.

Quality Improvement: The service coordinator should stay up-to-date on the latest research, trends, and best practices in disability services to ensure that the services provided are evidence-based and effective. Also, coordinators should participate in the ongoing review and evaluation of data relating to incidents to ensure continuous improvement and review of practices. ​Overall, the service coordinator for disability services is responsible for ensuring that individuals with disabilities receive the support and services they need to live a fulfilling life and achieve their goals. Essential Competencies and Experiences:

  • Demonstrated knowledge and experience in disability program management (assessment, coordination, community linking and budgets) and understanding the care needs of people within the disability sector;
  • Commitment to person centred care and quality of care;
  • Demonstrated ability to supervise and lead staff;
  • Demonstrated ability to plan, co-ordinate and organise own work and work of team members;
  • Demonstrated ability to develop a customer service culture amongst staff;
  • Ability to maintain professionalism, integrity and confidentiality;
  • Proven ability to plan, prioritise, meet deadlines and juggle competing demands, working in a fast paced and flexible environment;
  • Strong knowledge of the Disability Services Act 2006 (NSW) and prescribed requirements.

Qualifications:

  • Bachelor's degree in a related field, such as social work, human services, or psychology.
  • At least 1 year of experience working with people with disability.
  • Knowledge of disability-related laws and regulations.
  • Demonstrated awareness and understanding of issues confronting people with disabilities in a home or community setting.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Ability to multitask and manage multiple priorities.
  • Strong organisational and time management skills.
  • Ability to maintain confidentiality and adhere to ethical standards.
  • Valid driver's license and reliable transportation.
