An exciting opportunity now exists for a Diabetes Educator to join the North and West Remote Health team in Townsville.

As part of a supportive, diverse, and multidisciplinary team, promote optimal health and wellbeing outcomes for individuals and communities at risk of or affected by diabetes. Working with non-acute clients, you will liaise with other Health Professionals to oversee the overall case management of clients.

You will work with a diverse range of clients, set your schedule and have the chance to see the real difference your work makes as you enable rural communities to live and receive high-quality holistic care.

Core duties

  • Integrate diabetes self-management education with clinical care;
  • Utilise group education to assist with meeting individual needs;
  • Determine comprehensive self-management plans to maximise health outcomes; and
  • Raise the awareness of healthy lifestyles, disease and disability and other health-related issues.

Skills and experience

The right candidate will possess a degree or equivalent qualification in a health-related discipline and be able to provide high-quality diabetes assessment and to develop an effective ongoing management plan.

In addition, you will need:

  • A combined blue card & NDIS worker screening;
  • A current, unrestricted ‘C’ class Australian Driver’s Licence; 
  • The right to live and work in Australia; and
  • Credentialing or ability to be credentialed with the Australian Diabetes Educators Association is essential.
