About the opportunity:

Community Interlink (CI) provide access to support services for frail older people, people with disabilities and their carers. Funding is received from Goulburn Valley Health for Home Care Packages (HCP). This funding is used to provide case management and in home supports for consumers to maintain safety, choice, dignity and independence in their home for as long as possible.

This is a full-time position, Mondays to Fridays, with an Accrued Day Off per month. The role is based at our Broadford Office with some opportunity to work from home.

This position is responsible for providing Planning and Case Management services to our consumers.

The role includes, but is not limited to:

  • Developing Care Plans and Service Delivery Plans based on the consumer's goals and according to funding received, liaising with CI Service Support Coordinators as required
  • Provide information to clients and their carers regarding the range and types of services available, allowing them to make their own choices.
  • Negotiate the access to appropriate services utilising lateral thinking and creative problem solving where necessary.
  • Advocate on behalf of individuals in support of their rights and to ensure that individuals are aware of advocacy agencies that they may be able to access.

To be successful in this role, you will have:

  • A Tertiary qualification in an appropriate Health and/or Human Services field, or extensive, relevant experience.
  • Well-developed interpersonal, negotiation and communication skills, including the ability to initiate and maintain contacts with a broad range of service providers and community agencies.
  • Knowledge of the Case Management approach to service delivery, in a rural community health setting.
  • Demonstrated ability to work independently and as part of a multidisciplinary team.
  • Competent computer skills, specifically in Windows programs.
  • Be fully vaccinated for COVID-19.
