About the Role

The NDIS transition Support Officer role will be based at the Neami Hurstville site to deliver a program funded by the National Psychosocial Support Measure – Extended Transitional Arrangements for Commonwealth Community Mental Health Clients (NPSM2). This program aims to support consumers who have been engaged in the PHAMS and or PIR programs who are yet to complete the NDIS application process or who have been declined during the application process and are eligible for Continuity of Supports (CoS) funding. The primary focus of this service will to test NDIS eligibility for all consumers in the service who are yet to do so, transition consumers who receive NDIS plans to appropriate NDIS providers and support the remaining consumers with continuity of support until a Continuity of Support provider is established in the area.

Core duties

  • Engage consumers and develop trusting and professional relationships
  • Support consumers to test for NDIS eligibility
  • Maintain records and resources
  • Work with consumers to gather evidence to demonstrate psychosocial functional impairment
  • Ensure transition to appropriate NDIS services

What will you bring to the role?

  • Relevant experience in a similar role
  • Ability and willingness to work effectively as part of a team, to meet program goals and objectives.
  • Knowledge of complex mental health and drug and alcohol issues.
  • Knowledge of the NDIS application process
  • Highly developed written and verbal communication skills
  • Be a team player
  • Knowledge of the recovery model

  • Current Working with Children Check
  • Australian Working Rights
  • Drivers licence
  • Police check (refer to PD) – disclosable outcomes considered
