About the Role

• Full Time, Maximum Term Contact until 30 June 2019
• Based in Neami Thomastown or Fairfield
• Secondment opportunities available for internal applicants

The Support Facilitator will be part of the Northern Melbourne Partners in Recovery Program (PIR). The PIR program aims to support consumers with a severe and persistent mental illness, who also have many needs, and their carers and families to access services and supports from multiple sectors. The role of the Support Facilitator is to coordinate, facilitate and maintain access to the required services.

Core duties

• Provide a coordinated service to consumers with complex needs by working collaboratively with organisations and clinical case managers to ensure seamless access for consumers
• Engage consumers and develop trusting and professional relationships
• Maintain records and resources
• Collect, collate and maintain data on consumer contact

What will you bring to the role?

• Solid experience in the human services environment
• Demonstrated understanding of how best to access and maintain services for consumers
• Ability and willingness to work effectively as part of a team, to meet program goals and objectives
• Commitment to supporting recovery and improving outcomes for people living with mental illness

• Highly developed written and verbal communication skills

• Current Working with Children Check
• Australian Working Rights
• Drivers licence
• Police check (refer to PD) – disclosable outcomes considered
• A Disability Worker Exclusion Scheme Check will be undertaken prior to commencement
