Continuity of Support (CoS) provides non-clinical psychosocial support to consumers who previously accessed the Partners in Recovery (PIR), Support for Day-to-Day Living in the Community (D2DL) and Personal Helpers and Mentors (PHaMs) programs. The service supports people with complex mental illness who have been assessed as ineligible for the NDIS.
Services delivered under CoS are predominantly group-based and focused on building individual capacity, with additional targeted one-on-one support at times of increased need.
CoS is delivered by Wellness Connect, a consortium of mental health and community organisations led by Neami National and comprising Mission Australia, UnitingCare Wesley Bowden, Life Without Barriers and Community Access and Services SA. We work together towards a common purpose: to improve the health and wellbeing of people experiencing mental ill-health.
Wellness Connect are recruiting for two roles to be situated within Neami National and Life Without Barriers.
As an NDIS Access Officer-CoS within Wellness Connect, your role will be to review Continuity of Support Consumers for potential eligibility to the NDIS and assist those selected to test their eligibility with the NDIA. You will also contribute to the transition of consumers to NDIS providers where eligibility and access is granted.
