About the Role

  • Full Time (Part Time 0.8 FTE considered) positions
  • Maximum Term contract until 30 June 2023 (strong possibility of extension)
  • $74,253 - $79,890 p.a, pro rata + Super + Salary packaging + Employee Benefits!
  • Based at Neami Fairfield and/or working mobile with approval

The Department of Families, Fairness and Housing (DFFH) is funding the MHCSS Intake service to deliver the Mental Health NDIS Access Project (the Project). The project commenced in June 2018 and is to conclude on June 30, 2023.

The Project is a time limited initiative that will support people with a severe mental illness and substantial, enduring psychosocial disability who meet the disability, age and residency requirements of the NDIS to collect and collate the evidence they need to make an NDIS access request.

Core duties

  • Accepting referrals from providers, individuals, carers seeking clinical evidence to complete the access process, undertaking a pre-screen to determine likely eligibility for the NDIS and offering support to complete the access process, as required.
  • Collecting and collating all necessary evidence and completing the Access Request Form
  • Assisting with submitting Access Request Form to the NDIA and acting as a contact person for NDIA queries
  • Waiting list management intervention which includes: call back, cross sector referrals, gathering additional information etc.
  • Perform risk assessments (including referral to crisis intervention services, where necessary).
  • Maintain accurate individual consumer files and data bases (Carelink)
  • Facilitation of small group wellbeing programs (as required)

Mandatory requirements:

  • Current Working with Children Check
  • Australian Working Rights
  • Drivers licence
  • Police check (refer to PD) – disclosable outcomes considered
  • A Disability Worker Exclusion Scheme Check will be undertaken prior to commencement
  • Neami National will require, as part of our condition of employment, that all candidates applying for any position must be fully vaccinated for COVID-19 (or have received one dose of an approved COVID-19 vaccine and have a scheduled appointment to receive their second dose.)
  • Candidates will be required to provide appropriate evidence of COVID-19 vaccination to Neami National.
