“Full citizenship for all people living with a mental illness in Australian society”
Way2Home was established in 2010, currently working with FACS, City of Sydney and specialist homelessness services to reduce homelessness in the city. The service is focused on working assertively with a vulnerable homeless population with the aim of transitioning people into long term sustainable housing with support.
Mental health issues are a common presenting issue within the homelessness sector. In some cases people first become homeless due to an untreated or poorly supported mental health issue. There are also people who have fallen into homelessness and developed mental health issues subsequently.
Primarily provide an initial identification of needs for individuals, families and carers when they contact. Operating out of a central intake hub, you will initially assess the nature and urgency of the request for supports/services
Utilise your advanced knowledge and assessment experience to establish eligibility of the individual, urgency and case prioritisation and whether previous services have existed in the past
Assess the need for supports such as intensive specialist mental health screening and perform risk assessments, including arranging referrals to crisis intervention services, where necessary
Carry out comprehensive assessments of the needs of the individual and provide the individual with the necessary information of the sorts of services available to support their identified needs and the possible waiting times for services and support programs.
The successful applicant will have experience and working knowledge of homelessness, demonstrate integrity, credibility and commitment to Neami values. Your ability to build rapport quickly and create supportive working relationships with people sleeping rough, assisting them to access support services they need will put you in good stead for this role. You will adapt and respond well to change, foster and maintain respectful professional relationships with consumers, staff and organisations alike. It will be important to be able to maintain a hopeful and positive outlook even during challenging times.
