About the Role
- Full Time, Maximum Term contract until 30 June 2022
- CSD Level 3 from $74,253 - $79,890 pa (pro rata) + super + salary packaging
Working in partnership with Western Sydney Local Health District, Neami delivers the Enhanced Adult Community Living Supports (CLS) program and The Way Back Support Service (TWBSS).
The CLS program provides community-based support to people living with a mental illness in Western Sydney as well as providing in-reach and transitional support to people leaving acute and long-term hospital admissions.
The Intake and Transition Coordinator (ITC) provides a streamlined pathway and coordination of all referrals in the CLS Program. The ITC coordinates the discharge planning processes for consumers in the inpatient units within the Local Health District, including acute and long term stays as well as liaising with local Community Mental Health Teams to assess the need for extra support in the community.
Core duties
- Work with consumers at commencement of acute admissions to identify discharge pathways
- Undertake intake and assessment of consumers in an inpatient setting
- Work with consumers to identify their needs, set goals and develop a plan to meet those goals
- Provide coordination of any in-reach activities in the in-patient units such as Flourish and the Health and Wellbeing Programs
- Develop detailed discharge and transition plans for consumers preparing to leave hospital and live in the community
- Attend partnership meetings as a Neami representative to provide updates on intakes and care planning for current joint consumers
- Play a key role in the allocation and coordination of work to Community Rehabilitation and Support Workers in the team to provide comprehensive discharge planning and transition to community living
- Play an essential role in maintaining key partnerships in the area and establishing inclusive referral pathways
- Provide direct support to consumers on an ad hoc basis or as required.
What will you bring to the role?
- Previous experience in a similar role desirable
- Ability and willingness to work effectively as part of a team, to meet program goals and objectives
- Knowledge of complex mental health and drug and alcohol issues
- Highly developed written and verbal communication skills
- Ability to efficiently source and organise resources required for a task
- Knowledge of the recovery model
Mandatory requirements
- Minimum Certificate IV in Mental Health or equivalent
- Current NSW Working with Children Check
- Australian Working Rights
- Drivers licence
- Police check (refer to PD) – disclosable outcomes considered
- Neami National will require, as part of our condition of employment, that all candidates applying for any position must be fully vaccinated for COVID-19 (or have received one dose of an approved COVID-19 vaccine and have a scheduled appointment to receive their second dose.)
