About Street to Home:

Neami Street to Home service provides culturally appropriate, assertive outreach and case management to homelessness people sleeping rough in the Adelaide CBD within a Housing First approach.

The assertive outreach model is aimed at providing integrated services that focus on consumer identified goals, improved health outcomes and transition from homelessness to safe, secure and sustainable long term accommodation. We persist and work with people over the medium to long term supporting people to maintain their tenancies once they are in housing.

Street to Home staff will be responsible for engaging individuals sleeping rough within the Adelaide CBD, North Adelaide and surrounding parklands. Community Rehabilitation and Support Workers (CRSWs) and Street Outreach Workers (SOWs) (terminology used interchangeably) are responsible for providing assertive outreach patrols to rough sleepers with the aim of providing integrated services that focus on consumer identified goals, improved health outcomes and transition from homelessness to safe, secure and sustainable long term accommodation. We persist and work with people over the medium to long term supporting people to maintain their tenancies once they are in housing.

About the Role

  • Multiple roles available including FT, PT and Casual roles
  • 7 Day Rotating Roster where shifts start and finish times may change based on seasonal factors (some requirement for 6am shift starts or 9pm shift finishes)
  • CSD Level 2 from $61,529 - $66,110 per annum (pro rata); Casual employment $31.13 - $33.45 p/h plus casual loading
  • Maximum Term Contract until 31 December 2019; Casual employment has no fixed days
  • Work in a multidisciplinary team of health professionals and support workers
  • Provide a range of rehabilitation and support to consumers, tailored to meet their individual needs whilst working within the framework of the Street to Home service model
  • Work closely with consumers to address housing and immediate needs

Mandatory requirements:

  • Must provide Neami evidence of a Mental Health (or equivalent Human Services or Community Services) qualification of Certificate IV level or above, or complete such a qualification within 2 years of your commencement of employment
  • Current Working with Children Check and Vulnerable Person check required before commencement of work (employee responsibility)
  • Australian Working Rights
  • Drivers licence
  • Police check (refer to PD) – disclosable outcomes considered
