The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).

Are you a Social Worker looking for your next career move? On offer, is an exciting opportunity for a Senior Social Worker working within Primary Care and Community Health on a Permanent Full Time basis. This position will work out of our Lemongrove Campus.
In the role of Senior Social Worker, you will be required to provide a high quality, specialist Social Work service to clients/carers/family of the Primary Care and Community Health (PC&CH) services: Chronic and Complex Care Service (CCS), Transitional Aged Care Program (TACP) and Older Persons Evaluation, Review and Assessment Service (OPERA).

This position will contribute to identifying short and long term client goals, as well as the planning and implementation of social work interventions in conjunction with allied health, nursing and support staff. Advanced professional judgement will be required to develop an initial care plan which identifies the need for social work intervention, the optimal time frame in which interventions should be delivered and the most appropriate delivery option for the required interventions.

Selection Criteria:

  1. Degree in Social Work from an accredited course providing eligibility for membership of the Australian Association of Social Workers and a commitment to maintaining a professional skill and knowledge base. Overseas qualifications must supply the AASW Overseas Assessment number
  2. Current NSW State Driver Licence, including P1 or above and willingness to use for work purposes. This may include driving long distances across NBMLHD.
  3. Demonstrated extensive postgraduate clinical experience in the delivery of Social Work services, with a demonstrated advanced ability to analyse problems, set priorities and apply a range of options to support client goal setting and care plans. Extensive specialist knowledge with adult clients who have chronic and complex health conditions and of the community services relevant to this group. Ability to demonstrate advanced reasoning skills and operate autonomously with minimal direct clinical supervision. Ability to apply professional knowledge and judgement when performing novel, complex or critical social work tasks.
  4. Demonstrated ability to work collaboratively as a member of a multidisciplinary team to create a holistic and cohesive approach to service delivery.
  5. Demonstrated experience in staff supervision, as well as experience in the clinical supervision of undergraduate Social Workers.
  6. Commitment to professional development and training and willingness to engage positively in clinical supervision and performance development processes.
  7. Demonstrated high level verbal, written and interpersonal skills and the ability to interact constructively and collaboratively with a diverse range of clients and stakeholders throughout all organisational levels.
  8. Capacity to provide advice to service managers on clinical service development, practice and redesign.
