Nepean Hospital is a teaching hospital that provides tertiary referral services in emergency care, coronary care, diagnostics, gynaecology, paediatric, neonatal intensive care/ICU, maternity, mental health, rehabilitation, and surgery. On offer is an opportunity for a Registered Midwife to join the Post Natal ward in the role of Discharge Planner. The assignment will cover for Maternity leave.

The Discharge Planner provides a link to services, both internal and external, for women and babies to ensure appropriate follow up of postnatal care. In this role, you will aim to assist in ensuring a fluent process for women and their babies for up to 14 days post birth from hospital discharge to the community.

The incumbent will work collaboratively with members of the multidisciplinary health team utilising effective team communication in the planning, implementation, assessment and evaluation of patient care. Selection Criteria:

  1. Current registration with AHPRA Registered Midwife (Division 1)
  2. Demonstrated ability to practice in accordance with the Registered Midwife National Competency Standards, Code of Ethics for Midwives, and Code of Professional Conduct for Midwives (Nursing and Midwifery Board of Australia)
  3. Recent competent clinical experience in midwifery postnatal care and lactation support with a proven ability to make clinical assessments of postnatal women and neonates.
  4. An understanding of woman centred care and demonstrated ability to work effectively both independently and as part of the multi-disciplinary team.
  5. Demonstrated ability to lead, supervise, delegate, and negotiate with others to deliver quality midwifery care.
  6. Demonstrated ability to effectively prioritise and organise workload and deliver care with integrity and accountability.
  7. High level of written and verbal communication skills and demonstrated computer literacy with eMR and eMaternity systems.
