My Place

My Place is a not-for-profit provider of individualised and flexible supports to people with disability and their families. My Place supports over 400 people with disability to live in their own homes, or remain in their family home, and to become valued and contributing members of their community. My Place does not provide any group home, day centre of other congregate care arrangements. We support consumers in Perth Metropolitan Area, Great Southern region and the South-West region.

We are currently seeking applications for a full-time Co-ordinator in the metropolitan area to work in the Family Services Team. The Family Services Team works with children and young people with disability who are in the care of the State Child Protection system.

The Co-ordinator will be responsible for supporting My Place Consumers working in collaboration with Child Protection Family Services - Department of Communities, to engage and manage appropriate supports.

The successful candidate will be offered an excellent salary package, the opportunity to salary sacrifice, access to a meals/entertainment card, five weeks’ annual leave and other allowances. Whilst this work is primarily community based, the successful applicant will be required to work from a home office for which a subsidy will be paid.

Selection Criteria

Required:

  1. Demonstrated contemporary attitude and values toward people with disability.
  2. Experience in contemporary and innovative service design for individuals, and the implementation and ongoing management of individual support arrangements.
  3. Demonstrated well developed interpersonal, communication and written skills, including in the context of supporting consumers and team and staff supervision.
  4. Experience in developing and managing individualised budgets.
  5. Competent in the use of standard computer packages and information technologies.
  6. Ability to work autonomously from home office, in the community or office.
  7. Has a current Driver’s Licence and WA Police Clearance. Co-ordinators will also be subject the National NDIS worker screening process post 1st December 2020.

Desirable:

  1. Previous experience in supporting young people with trauma and/or mental health issues.
  2. Demonstrated understanding and respect for Aboriginal and Torres Strait Islander cultures, histories, knowledge, and rights.
  3. Knowledge of and/or experience in planning, claiming and review processes within an NDIA environment.
