About the Role
In this community‑based nursing role, you will provide safe, competent and holistic care within your scope of practice. You’ll work closely with Registered Nurses and contribute to positive health outcomes for clients across a defined geographic area.
Key Responsibilities
- Deliver personal care services and supports, including transport assistance
- Identify and report changes or deterioration in client condition
- Monitor vital signs such as weight, blood pressure and other observations
- Obtain and store medication when authorised
- Provide specialised nursing support where trained (e.g. catheter care)
- Assist the RN team with palliative care
- Respond appropriately to incidents, injuries or emergencies
- Administer medication when trained and authorised
- Support clients with medication self‑administration
- Provide domestic assistance as required
- Support nutrition, hydration, mobility and independence
- Maintain accurate documentation and progress notes
- Uphold confidentiality and privacy standards
- Follow infection control and Work Health & Safety requirements
What We’re Looking For
Mandatory Requirements
- Certificate III Aged Care (Individual Support)
- Current First Aid Certificate
- Current NSW Driver’s Licence
- Strong teamwork skills
- Excellent written and verbal communication, with Microsoft 365 experience.
- Commitment to quality and continuous improvement
Desirable
- Experience in community nursing
- Aged care experience
- NDIS experience
Other Requirements
- Commitment to the Aged Care Code of Conduct
- Eligibility to work in Australia
- Maintenance of all required registrations and competencies
- Willingness to undertake tasks within your skills and training
