Our Journey

Monash Health National Disability Insurance Scheme (NDIS) Program supports people who have a disability, to help them in their daily life, participate in the community and reach their personal goals. The NDIS program comprises of support coordination, allied health services for both children and Adults. The NDIS Team operates out of several Monash Health Community sites including Springvale, Cranbourne, Pakenham, within participant’s homes as well as providing supports within community facilities.

A Day in the Life

An exciting opportunity has become available for an enthusiastic and experienced Team Leader of our Support Coordination Team. This is a great position for someone who has a passion for leading and work in a multi-disciplinary team who provide services to NDIS participants. This is a 4-day-a-week position with the opportunity for flexible working arrangement including working from home. You will be responsible for working within the Community NDIS Program to provide best practice clinical care, achieve maximal operational effectiveness, develop organisational competencies, and create strong partnerships within and outside the NDIS Program and Monash Health.

You will also be responsible for providing day-to-day leadership to the Support Coordination Team as well as having a clinical load supporting our NDIS participants, ensuring participants understand and are able to access the full benefits of their NDIS plans to achieve their life goals. This is position with in Monash Health is unique to the resources that a large public hospital has to offer.

What you will Bring:

As a leader you will lead, coach and work collaboratively with support coordination team members; identify, plan and coordinate required support services to achieve goals and meet KPI targets Report on the delivery of support coordination hours. Work with in a supportive and innovative team that is leading the way in providing excellent life outcomes for our participants. You will also bring:

  • Excellent communication and interpersonal skills
  • Demonstrated productivity including initiative, good problem-solving skills, judgement and attention to detail
  • Enthusiastic, compassionate & able to work collaboratively

What you need:

  • Knowledge and experience working as a Support Coordinator
  • Ability to work with participants to increase their capacity to maintain relationships, manage their supports and empower to live independently in the community.
  • Bachelor or Master's Degree and/or equivalent qualification relating to the field or relevant extensive experience working in the field.
  • Satisfactory NDIS Worker Screening Check
