We are now looking to appoint a permanent full-time and fabulous Grade 1 Social Work professional, at Monash Medical Centre in Medical Specialities area. As a member of the team you are integral to the Monash Health Social Work service delivery and consistently demonstrate our core values. You will be part of a flexible workforce with personalised support. You will be a member of a dynamic team with the opportunity to work in a setting assisting adult patient cohorts from general medicine, general surgery, Emergency department, Women’s and Children’s and other specialty areas as required. The focus is on psychosocial assessments, risk assessments, complex discharge planning and family support.
