The program has Five PARCS (Preventions and Recovery Care Services) which includes a specialist Women’s only PARCs, an Extended Care PARCs (EPARC) and Youth PARCS. The clinical service is supported by an aligned operational structure to enable the provision of high quality service through the creation of clinical-operational partnerships to support staff, consumers, carers and their families, and a strong professional structure that supports the governance of the health professional workforce.
We are seeking an experienced and enthusiastic Grade 2 Social Worker for our Prevention and Recovery Care Centre Service, based at Clayton PARCS. This is a permanent ongoing full time position.
This unique social work role will be located at Clayton PARCS and provide in reach into P Block – Psychiatric Inpatient Unit, along with the other community teams within the Clayton Mental Health Program. The role will sit between the Clayton bed-based social work team. And endeavour to achieve seamless transition for consumers and involve their families and carers when they move between the community, PARCS and/ or the inpatient unit, thus providing a discipline specific focus across to consumers moving from Clayton Inpatient Unit and the community teams into PARCS.
Clayton PARCS is a 10 bed short term residential facility where consumers can stay (2-4 weeks). The model is focused on assisting consumers to meet their identified recovery goals, whilst offering the consumer and carer’s respite. Referrals are received from inpatient units, community teams as well as external stakeholders (consumer/family/GP/Private mental health practitioners).
