An exciting opportunity has arisen for a Case Manager to join the Monash Health Community Rehabilitation Team.

Community Rehabilitation offers client-centred, goal driven rehabilitation services to maximise a client’s independence, health and well-being following acute illness or injury. Community Rehabilitation consists of large interdisciplinary teams that work with the client to achieve their rehabilitation goals and provide home and centre-based therapy.
The Community Rehabilitation Case Manager facilitates the successful transition of referred clients from hospital to home and toward community reintegration. The Case Manager completes comprehensive assessment and care planning in collaboration with the client and interdisciplinary team.
The Case Manager is the key contact to the client, team and key stakeholders in a variety of settings coordinating the care plan to ensuring high quality client-centred care. The Case Manager supports client flow within the community rehabilitation program.
This position is based within Cranbourne/ Pakenham Community Rehabilitation; however, travel to and work at any of the Monash Health sites will be required. The incumbent will be required to work in any clinical area at any or all campuses of Monash Health as and when required. Such a requirement may be a temporary arrangement, on an ad hoc basis, or a permanent change.
