Your Opportunity

Mission Australia Blacktown/Hills Family Support Service provides support for young families and young people across the Blacktown and Hills district LGA's who are homeless or at risk of homelessness to find appropriate, affordable and sustainable housing.

The service provides tenancy support, information and advice, referrals to other support services, and life skills development.

Through our services, we deliver a combination of practical support and guidance to help people turn their lives around.

Your key responsibilities will be to:

• Undertake initial assessments including all required consents and paperwork
• Work with clients to create individualized support plans including referral to supplementary services as needed.
• Provide ongoing case management sessions with clients and review progression against support plans regularly and provide informal counselling as required.
• Undertake outreach activities including rough sleeper patrols and service delivery at various located location within the supported community locations
• Monitor the transitional housing facilities to ensure that all aspects of property are maintained and address any areas that need attention
• Develop strong relationships with key external and internal stakeholders
• Provide outreach services at key first to know agencies, e.g. DCJ housing and Centrelink

Requirements for Success

• A Degree or Diploma in Social Welfare/Social Science or relevant industry experience
• Minimum of at least 1 years' demonstrated experience in case management
• Experience working with families with complex needs and understanding of DCJ Families and housing
• Experience in Child Protection
• Ideally a background in working with homelessness and social housing
• Experience in the use of online client management systems
• A current NSW Driver's License

Before starting work with us, you will need to undertake a national police history check, qualifications and referee checks, and have a clearance to work with children/vulnerable people.
