Your Opportunity

Mission Australia currently have opportunities to join us as a Youth Case Manager for the Northern Sydney Youth Homelessness Service across the Northern Sydney District. This position will provide support to young people aged 12 -24 who are 'at risk' and require practical and supported intervention to enhance safety and connection.

The case manager will provide assistance to young people through client centred individual support focused on exploring safer accommodation options (where required), enhancing positive educational/workforce engagement, address mental health/AOD concerns and provide referral options to other appropriate services. You will be passionate about empowering young people to remain in safe and supportive environments whilst encouraging connection to supports within the Community for ongoing independence and stability. You will also be a good collaborator, actively engaging with the local service sector and support young people to engage with the Rent Choice Youth program (DCJ Housing).

Your key responsibilities will be to:

  • Respond to referrals to the service, and conduct thorough assessments to determine their eligibility and support needs and establish a case plan with the client and the other relevant stakeholders
  • Contribute to the development and facilitation of group sessions to meet the needs of the service, clients and the local community
  • Develop, implement and review individualised case plans for each client, addressing such issues as living skills, welfare needs, interpersonal skills, health, education, training, employment options, social skills, recreational needs and family relationships
  • Establish record, monitor and review each person's progress, in consultation with Program Manager
  • Develop quality relationships with a range of key stakeholders including community organisations, rehabilitation providers and other health professionals to allow for the effective provision of support and information to clients

Requirements for Success

  • Relevant degree or diploma with at least 2 years' experience
  • Demonstrated ability to provide specialised or complex therapeutic interventions and manage a caseload of complex Consumers
  • Experience with clients dealing with issues including homelessness, poverty, abuse, mental health and the use and misuse of alcohol and other drugs, and knowledge of support services
  • Highly developed administrative skills along with well-developed written and oral communication
  • An understanding from combined lived experience and professional development about factors that support recovery and a proven ability to use this to positively support others on their recovery journey

Before starting work with us, you will need to undertake a national police history check, qualifications and referee checks, and have a clearance to work with children/vulnerable people.
