Mind Australia is one of the country’s leading community-managed specialist mental health service providers with a range of residential, mobile outreach, centre based and online services. We have been supporting people living with the day-to-day impacts of mental illness, as well as their families, friends and carers for over 40 years.
We provide practical and motivational support that helps people to develop the skills they need to move on, thrive and improve the quality of their lives. It’s an approach to mental health and wellbeing that looks at the whole person in the context of their daily life. Mind is committed to diversity and social inclusion.
The Fee For Service (FFS) program offers a range of specialist interventions and approaches to people with psychosocial disability and complex needs who have NDIS plans and/or TAC and WorkCover support packages with the focus on maximising recovery outcomes in a fee for service model. The FFS program provides centre based and tele-health services, outreach services, support coordination, and education and vocational support. The service offers clients choice and control over services so they are able to build life skills and capacity, develop social connections and create community linkages to identify and achieve their recovery goals.
We are seeking a self-motivated, compassionate Support Coordinator for a fixed term, part time (22.8 hours/week) opportunity to work alongside a dynamic team. Working from home, you will provide Level 2 Support Coordination to participants with complex needs to initiate and coordinate their NDIS plan, negotiate appropriate support, access a range of mainstream services and community support and develop their skills and confidence to manage their plan independently. In this role you will apply the principles of choice and control by ensuring awareness and options of services are offered to the participants.
