The Yandina Transitional Housing and Mental Health Accommodation Pathways at Discharge (MHAPD) outreach services sit within Mind’s Sub-Acute program and operates as a client-centered framework providing services to those with severe mental illness. These services provide recovery based interventions aimed at successfully supporting clients to address and overcome barriers that contribute to homelessness or increased risk of homelessness.
We are seeking a passionate, motivated permanent, part time (23 hours worked over 4-5 days a week) Community Mental Health Practitioner - MHAPD in the Wangaratta region. You will work autonomously with an empathetic, collaborative team and provide outreach services to clients to reduce the impact of homelessness within the community.
