The Country Wellness Connections (CWC) program provides psychosocial support services and community connection to people experiencing mental ill health in country South Australia. The CWC service delivers a person-centered approach to recovery and a range of tailored non-clinical community-based supports, mental health counselling and improved access to the NDIS to help them get on with their lives. Clients are supported to enhance connections with community services, engage in social activities and community groups, build capacity and daily living skills, develop confidence to live independently, achieve recovery goals and improve quality of life.
We are seeking a compassionate, self-motivated Community Mental Health Practitioner for a 3 month fixed-term, part time (15.2hours per week) opportunity to in the Riverland area. You will deliver positive person-centred outreach services to clients to achieve their wellbeing and recovery goals, build social and life skills, strengthen relationships with family and friends, enhance support networks, and capacity to live independently in the community.
