The From Homelessness to a Home (H2H) program, in consortium with Launch Housing and other specialist agencies across metropolitan Melbourne, is a Victorian Government initiative to assist people experiencing homelessness to transition from temporary accommodation into housing. H2H aims to make a significant, lasting impact on homelessness and rough sleeping by supporting clients with access to rental properties and tailored mental health, AOD, family violence, and physical healthcare services based on individual need.
We are seeking compassionate, resilient Community Mental Health Practitioners for 7 month fixed term, full or part time opportunities to make an impact on homelessness in the Melbourne metro area. Working from home, you will deliver positive recovery-orientated outreach services to clients to transition to affordable housing, increase their health and wellbeing to prevent a return to rough sleeping. As this position requires travel across the Melbourne metro area a fully maintained company car is provided.
