Position Context

MIC receives funding from the Eastern Melbourne Primary Health Network (EMPHN) to deliver the Care Finder Program in the Local Government Areas of Manningham, Monash and Whitehorse.

The Aged Care navigation program will provide intensive support for people aged over 65 years and their carers who have difficulty accessing and maintaining aged care services themselves.

Reasons for requiring intensive support may include:

  • isolation or no support person (e.g. carer, family or representative) who they are comfortable to act on their behalf and/or who is willing and able to support them to access aged care services via My Aged Care
  • communication barriers, including limited literacy or digital skills
  • difficulty processing information to make decisions
  • resistance to engage with aged care for any reason and their safety is at immediate risk or they may end up in a crisis situation within (approximately) the next year
  • past experiences that mean they are hesitant to engage with aged care, institutions or government.

The program provides individual support to help people to remain living at home independently and/or to access services that best meet their needs, as well as identifying gaps in service provision and developing appropriate responses. The MIC’s Program is one of 6 programs in the Eastern Metropolitan Region (EMR).

Key Duties

  • Provide assertive outreach to promote the program to older people from CALD backgrounds e.g. promotion through health networks, GPs, CALD senior’s groups, etc.
  • Build rapport and trust with older people from CALD backgrounds who are eligible for aged care services but are reluctant to access aged care services or who need intensive support to do so
  • Provide information and support to older people aged over 65 years to assist them to access services including supporting them to interact with My Aged Care (MAC) so that they can be screened for eligibility for aged care services and referred for assessment
  • Providing support to clients after they have been linked to services to ensure that services are meeting their needs and they are engaged with services
  • Develop effective links with CALD communities and individuals and the services that support them in Manningham, Monash and Whitehorse
  • Provide information about aged care and the broader service system for diverse communities and individuals, including facilitating information sessions and forums for groups
  • Work collaboratively with Commonwealth Home Support Program (CHSP) funded services to facilitate and promote improved client access and support to meet the needs of FACS clients
  • Complete all client recording and reporting requirements

Key Selection Criteria

Essential

  • Experience in working in the community sector in a direct service provision role supporting people from diverse backgrounds
  • An understanding of diversity issues as they relate to individuals and their engagement with community, government and health services
  • Excellent interpersonal skills and demonstrated experience in delivering client focused services
  • Demonstrated experience in working collaboratively with a range of service providers in the community sector with a specific focus on aged care support services
  • Highly developed written and verbal communication skills
  • Excellent computer skills
  • Demonstrated ability to apply a practical solution-focused approach to problem solving.
  • Current driver’s license

Desirable

  • Knowledge and experience in working in Aged Care services in Victoria, including experience working with My Aged Care and knowledge of the Home Care Common Standards
  • Familiarity with the Home Care Common Standards
  • Tertiary qualification in Social Work, Community Services, Community Welfare or other relevant discipline
  • Ability to speak a language other than English

Position dimensions

  • All offers of employment at the MIC are subject to three-month probationary period, a satisfactory Police Check and Working with Children’s Check and the applicant having had at least two doses for COVID-19 vaccination prior to commencement of employment
  • All MIC staff work in co-operation with other staff and volunteers of the MIC
