Micah Projects is a community based not-for-profit organisation with a vision to create justice and respond to injustice at the personal, social, and structural levels in church, government, business, and society. The Care Finder Program is a part of consortium comprised of Micah Projects, Communify Queensland and Queensland Positive People that targets people who have one or more reason for requiring intensive support to interact with My Aged Care, access aged care services and/or other relevant supports in the community.

As an Aged Care Navigator, you will work as part of the Service Integration and Health Cluster to advocate for and support individuals so they are empowered to access appropriate resources, opportunities and aged care services within the community.

This is a full time, fixed term employment opportunity until 30 June 2025, located at West End, with conditions and salary at Level 4 of the SCHADS Award.

We are seeking candidates with the following skills & experience:

• Relevant experience and/or appropriate qualification including certificate, diploma, and tertiary qualification according to level of responsibility.
• Demonstrated knowledge and experience of working with people who have experienced disadvantage.
• Demonstrated engagement and interpersonal skills, using a non-judgemental approach.
• An understanding of assertive outreach and trauma informed care.
• Demonstrated ability, and commitment, to work collaboratively within a team and with external stakeholders.
• IT capability including the use of MS Office Suite, Case Management and Data Systems.
• Demonstrated commitment to professional practice through continuing professional development.
• Demonstrated commitment to upholding child and vulnerable adult safeguarding principles.
• A positive attitude, and the ability to work flexibly and proactively in a team environment.
• Demonstrated commitment to cultural diversity and a working knowledge of equity and WHS principles in the workplace.
• Commitment to safeguarding children and vulnerable adults.
• Commitment to cultural diversity, natural and social justice.
• Current criminal history check and valid working rights in Australia.
