ABOUT THE ROLE

We’re looking for two skilled and compassionate Case Managers to join our Low-Cost Accommodation Support Program. Working across Brunswick and Northcote, you’ll deliver on-site and outreach-based support to vulnerable adults and older people living in low-cost accommodation or public housing.

Many of the people you’ll support are experiencing complex health and psychosocial needs, isolation, and housing insecurity. Through short-term, goal-directed case management, you’ll assist clients to sustain tenancies, access health and social services, and reconnect with their community.

This role forms part of a vital suite of older persons programs at MOSS, which includes the Community Connection Program (CCP), Housing Support for the Aged Program (HSAP), and the Older Persons High Rise Support Program (OPHRSP).

What we are looking for

We’re seeking values-aligned practitioners who are:

  • Experienced in case management or similar roles, ideally working with older people or those facing multiple barriers to service access.
  • Skilled in engaging people through assertive outreach approaches.
  • Knowledgeable in housing and homelessness systems, aged care, and disability services.
  • Strong communicators, with the ability to advocate, collaborate and navigate complex service systems.
  • Committed to social justice, cultural safety, and continuous improvement.
