Merri Outreach Support Service (MOSS) is a well-established, community-based organisation committed to ending homelessness, sustaining housing, and strengthening communities. Since 1989, we’ve provided responsive, inclusive and person-centred support to people at risk of or experiencing homelessness across northern Melbourne.

We’re looking for two skilled and compassionate Case Managers to join our Low-Cost Accommodation Support Program. Working across Brunswick and Northcote, you’ll deliver on-site and outreach-based support to vulnerable adults and older people living in low-cost accommodation or public housing.
Many of the people you’ll support are experiencing complex health and psychosocial needs, isolation, and housing insecurity. Through short-term, goal-directed case management, you’ll assist clients to sustain tenancies, access health and social services, and reconnect with their community.
This role forms part of a vital suite of older persons programs at MOSS, which includes the Community Connection Program (CCP), Housing Support for the Aged Program (HSAP), and the Older Persons High Rise Support Program (OPHRSP).
