We have an exciting opportunity for a HACC- PYP Linkages Packages Program Senior Case Manager to join our existing dynamic, dedicated & growing team. 

Based on the Mornington Peninsula, with offices in Mornington, Frankston, Casey and Cardinia, Mentis Assist provides support for people with diagnosed mental illness in a range of different programs.

The HACC –Program for Young People supports people under the age of 65 or Aboriginal or Torres Strait Islander people under the age of 50 with complex care needs who would benefit from case management to help coordinate their health care needs. The program seeks to build participant’s capacity and independence, while respecting their diversity of culture and needs.

Successful applicants will have tertiary qualifications (minimum diploma level) in mental health, social work, occupational therapy, nursing or community services, and a minimum of 2 years’ experience working in the areas of mental health, HACC, disability or similar. The ability to manage a complex case load of up to 20 clients and oversee brokerage and individual packages and service needs must be demonstrated in a similar role.

This position is based at Mornington and is classified at SCHCADS Level 4- 5 pending qualifications and experience.
