The Home Care Manager will take accountability for:
- Leading, growing, and shaping the foundations of our home care service to ensure it meets the requirements of ongoing changes in the sector
- Support the home care team to deliver a high quality, client focused home care service
- Day-to-day management of the home care service, including taking overall accountability for operational issues such as staffing, consumer care needs, physical resources, and finances.
- Ensure that care objectives for all consumers are developed, taking into account the individual’s needs, choices and funding available.
- Partner with clients, families, and community stakeholders to create a supportive environment where holistic care is prioritised.
- Plan and organise the effective use of time and resources for both current and future requirements ensuring the provision of quality care and services.
- Maintain compliance with the Aged Care Act accreditation standards for home care by ensuring areas of delegated responsibility are effectively managed.
- Manage the financial performance of the home care service through ensuring consumers can access services in a mutually cost-effective manner that allows for a safe, inclusive, and high-quality service.
The successful Home Care Manager will be able to demonstrate the following:
- Demonstrated experience in a Home Care Manager or Community Service Manager role
- Proven clinical knowledge and experience in aged care and Aged Care Quality Standards
- Understand the changes relating to Support at Home program and implement them into our Home Care service
- Strong interpersonal skills to lead, guide, and collaborate with the immediate and wider team to bring together available expertise and resources
- Demonstrated commitment to customer service;
- Strong computer literacy to effectively navigate and use various IT systems
Essential Criteria
- Current National Police Clearance (no older than six months);
- Drivers' License and ability to drive independently across our 3 sites in the Perth metro
- AHPRA Nursing Registration
