This is a Full-time Fixed (until 22/05/2023) opportunity based in Warrnambool, VIC within our Home Based Care Program.
MacKillop’s Out of Home Care (OOHC) Division provides home based care and case management support for young people with high and complex needs, and a therapeutically oriented program for young people in protective placements.
This position is situated within Home Based Care Wimmera South West Warrnambool and surrounding area, this position reports to the Home Based Care Coordinator.
This role is responsible for case management within a therapeutic and trauma informed model of care. It is responsible for supervision of caregivers, monitoring of placements, caregiver assessment, contribute to overall service development and commitment to best practice in Occupational Health and Safety.
