Interactive Community Care (ICCare) was established in 2010 as a specialist in the Community Services Sector and has now proudly served the people of NSW for more than a decade. A multidisciplinary community care organization, we provide out-of-home care, crisis support and disability services. Supporting families across NSW in the Hunter, Central Coast, Sydney, Mid-North Coast and Western regions, Interactive Community Care is committed to meeting the complex care needs of children, adolescents, and adults.
Lumiere Foundation is on the look out for an experienced NDIS Support Coordinator for work with ICCare's participants. You will provide varying levels of support coordination and transitions of supports for participants within their NDIS plan.
