About the role

Working within our Aged Care services team. the position on Team Leader will be responsible for overseeing the delivery of support services to ageing clients, clients with complex health needs and those requiring post hospital care and support in the community.  

This is a Permanent Full Time position, based in Goulburn/Upper Lachlan Shire. Whilst it is anticipated that the days of work for this role will be Monday – Friday, please note that flexibility to work after hours and on weekends (including on-call) may be required in order to meet client support needs.  

Key Responsibilities

• Coordinate and manage the daily operations of support services for aged clients and clients requiring post hospital care.
• Establish and maintain positive relationships with stakeholders including clients, families and guardians.
• Assess, develop and regularly review of client care plans, service agreements and available client documentation to ensure services are being delivered as agreed with client and/or their authorised delegate.
• Undertake regular reporting and documentation for both internal and external stakeholders such as government funding bodies.
• Provide direct support, care and assistance to clients as required to meet their support needs.
• Provide ongoing guidance, mentoring and training to a team of Home Care Workers.

Skills & Experience

• Certificate IV Community Services, Health, Nursing or similar.
• Previous experience in leading and/or mentoring a team utilising positive and proactive approaches. Prior clinical care experience within aged care, community, or health would be highly advantageous.
• Demonstrated experience supporting clients who are ageing and/or have complex health needs.
• High level written and verbal communication including reporting, stakeholder liaison and negotiation skills.
• Current Drivers Licence & First Aid certificate.

Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
